Accent At-Home Health Assessment

Convenient at-home blood tests
Our at-home blood testing is quick and easy to do from the comfort of your own home.

Blood pressure monitoring
You’ll receive a blood pressure monitor to keep and use for regular ongoing monitoring. Our GP will ask for your most recent reading during a follow-up call to support your overall health assessment.

Health & lifestyle assessment questionnaire
This focused assessment combines key physical measurements, like BMI, with lifestyle questions to give a clear picture of your overall health and well-being.

Cardiac risk score
This assessment estimates your risk of developing heart disease based on key factors like age, cholesterol, blood pressure, and lifestyle. Our GP will review your score with you and recommend any steps to help reduce your risk.

At-home cancer checks
Get to know your body and look out for changes like lumps, skin changes, or unusual symptoms. Our GP can offer advice on how to check at home and what to watch for in your own time.

Patient health questionnaire (PHQ)
The PHQ is a simple tool used to screen for mental health conditions and assess symptom severity. Our GP will review your answers, provide guidance, and connect you to the right care if needed.

What happens at your health assessment?

Request Your Kit
Click the provided link and complete the online request form. Our booking team will then send a blood test kit and blood pressure (BP) monitor to your home address.

Complete and Return Your Blood Test
Follow the instructions to complete your blood test at home, then return it to the lab using the pre-paid envelope provided.

Book Your GP Appointment
Once your blood test results are received by the lab, our customer service team will contact you to arrange your follow-up GP consultation.

Confirm Appointment & Complete Lifestyle Questionnaire
You’ll receive an email confirmation of your GP appointment, along with a link to a lifestyle questionnaire. Please complete this before your consultation. This includes submitting a blood pressure (BP) reading taken using your at-home monitor.

Attend Your GP Consultation
During your appointment, the GP will review your blood results and BP reading, conduct a physical assessment (via questions), assess your cardiac risk, guide you through self-checks, and complete a Patient Health Questionnaire (PHQ). Your GP will provide a comprehensive summary of your current health and recommend any necessary next steps.

Receive Your Personalised Health Report
After your consultation, you’ll receive a personalised report summarising your results and recommended actions to support your onward health journey.
