1. The Patient Story Promotion invites selected Vista Health patients (“Approved Group”) to complete a short survey about their experience with Vista Health.
2. Eligibility to participate and receive any associated rewards is limited to individuals who have been directly contacted by Vista Health and formally invited to complete the survey.
3. The promotion is open from 27 June 2025 and will close at 09:00am on 3 July 2025 (the “Promotional Period”).
4. All completed surveys submitted during the Promotional Period will be entered into a prize draw for the chance to win one of three £50 Amazon vouchers.
5. Winners of the prize draw will be randomly selected after the closing date and contacted using the details provided within 10 working days of the closing date.
6. A small number of participants may also be contacted to discuss the opportunity to feature in Vista Health marketing materials, such as written case studies, video interviews, and/or press features.
7. If a participant’s story is selected for potential featuring, they will be contacted by a member of the Vista Health team. Participation in any further marketing activity is entirely voluntary.
8. As a gesture of appreciation, individuals selected and who consent to take part in featured marketing content will receive an Amazon voucher valued between £100 and £250. The value awarded will be determined at Vista Health’s sole discretion, taking into account the nature of the participation, including responses provided to Question 7 of the survey ("Would you be open to sharing your story in a video or featured quote to help inspire others?").
9. Vouchers have no cash alternative and are non-transferrable.
10. The offer is only valid for the Approved Group and may not be shared or transferred unless written consent has been provided by Vista Health.
11. Vista Health reserves the right to verify all submissions and to withdraw, vary or amend this promotion at its sole discretion.
12. Vista Health accepts no responsibility for incomplete, inaccurate, or late submissions.
13. Personal data submitted as part of this promotion will be processed in accordance with Vista Health’s Privacy Policy.
